Patient Forms

During your first visit, a Wellness Medical Center staff member will greet you and ask you to fill out new patient registration forms. We recommend patients arrive 15 minutes before their appointment to allow time to complete the forms. Please feel free to download, print, and complete these forms ahead of time and bring them with you to your appointment.

Insurance

Wellness Medical Center accepts most major health insurance and auto insurance.

The services you seek might be covered in the network or out of network, depending on your insurance company and plan. Our office staff will verify your insurance and coverage for the services you seek. If you have any questions about billing or insurance, please call us at (617) 282-8700.

If you have been in an auto accident, you do not need a referral from another doctor to see a physical therapist.  We can see you immediately, and we may be able to begin treatment right away.

Courtesy Transportation

Wellness Medical Center provides free transportation from your home or works place to our clinic and back. If you would like to schedule free, round-trip transportation, please contact our office at least 24 hours prior to your scheduled visit by calling us at (617) 282-8700.

Do I need to make an appointment?

Yes. Often your referring physician’s office will assist you in making an appointment at our clinic. Or you can make an appointment by simply calling our office at (617) 282-8700.

What should I bring to my first visit?

Please bring the following to your appointment:

  • Health insurance card
  • Photo identification such as a driver’s license

What should I wear?

We want you to be comfortable during your physical therapy appointment. We usually recommend patients wear loose-fitting exercise clothing and comfortable shoes.

Can I fill out my patient registration information forms before my appointment?

Yes. You can download your patient registration forms from this Web site. Don’t forget to print them, fill them out, and bring them with you!

Of course, you can also fill out your patient forms when you arrive at our clinic. If you do, please arrive 15 minutes before your scheduled appointment to allow time to fill out the forms.

I don't have a car, how do I get to Wellness Medical Center?

We are conveniently located near Red line train and and bus routes. We also offer courtesy, round-trip transportation from your home or work place to our office. Please call our office for more information or to schedule free transportation at (617) 282-8700.

Do I need a physician's referral for physical therapy?

No. You may schedule an appointment without a physician’s referral for treatment.

Can I get insurance supported treatment without a referral from a doctor?

Yes. You can get insurance supported treatment without a doctor’s referral if your condition is for a condition related to an auto-accident. Non-auto accident patients need a referral from their primary care physician.

Does Wellness Medical Center bill the insurance companies directly, or are patients responsible for doing that?

Wellness Medical Center takes care of all paperwork and bills insurance companies directly.
